Let's look at them one by one.
Usually, when you want to ask "Have you eaten yet?", "你吃饭了吗?" would sound the best. The 了 in this case is just a simple change of state 了, emphasizing the difference between "have not eaten" and "ate". The question 你吃了饭吗? doesn't sound very natural when it's standalone because usually the S. V. 了 O. structure is followed by some other sentences. However, it makes sense in the context where you're emphasizing the object 饭. In this case, 饭 is not "meal" anymore, it refers to "rice". So the question is asking, "Did you have rice?" or "Was what you were having rice?"
Now for the sentences 我敲了门,不过他不在 and 我敲门了,不过他不在. Both are correct in this context, and both sound natural. The first one emphasizes the action "knock on the door" is completed, and the second one focuses on the change of state---I didn't knock the door, and now I knocked it. By now you probably have noticed that 了 is very versatile in Chinese, and sometimes the meanings overlap each other. It especially can get a bit confusing when it involves an object.
