yeah you're right mate. and i know it isn't just used for how to do something. rather, just how.
as zhenlijiang,changye and i have pointed out it connotes the terms of how much. i may not get about that much within china, residing in chengdu, but still i've never heard anyone use that term in any other way where cost is the context. notice the answer includes how much and for how long.
i've used it for house rent negotiations in the past. some landlords want a years' rent and some only three months. 房子怎么租？

自行车怎么租 refers to how the bike is rented. is it rented by the hour/day/second etc

to me that means 'how to buy'.
i've never heard anyone use 怎么 to express how much(as in cost).
a common way of asking hao much in sichuan is to use 耗hao4 (verb) cost. understood and used outside sichuan as well.
宫保鸡丁耗钱？宫保鸡丁多少钱？
租自行车多少钱？租电凭车耗钱？电dian4凭ping2
凭 rely on, be based on. (a rely on electricity scooter).
however, 宫保鸡丁怎么卖， could be used to ask how it is sold according to and price. for example the restaurant owner may only sell in bulk.
a highly unlikely situatution, but if they are a catering firm then the sentence may have context.

common ways of asking someone whether or not they live close to work 你的家离公司远不远？ 你的家离公司很近吗？alternatively you could ask 你的公司里你家远不远？你的公司离你家很近吗？
the first two sentences indicate that the question is asked at work. the second two outside work.

从 is used to define time and distance between places in time.
also used with 起 when a situation arises(time)， or a point from which further discussion must begin.
这个问题要从国际经济化说起。this problem must be discussed from the point of economic globalistion.
离 is used to define distance or space between places.
我的中文水平差的离谱！

April 28, 2009 at 5:29 AM宫保鸡丁 was the first chinese dish that was taught to me at uni. like xuchen i prefer to 辣 it up a bit.
aside from hotpot, perhaps the most popular dish in sichuan is 回锅肉hui2guo1rou4, twice cooked pork.
this dish is best avoided by cardiovascular challenged culinary adventurers. the main ingredients being fragments of pork meat 瘦肉 attached to chunks of pork fat 肥肉 accompanied by spring onions 小葱 and blackbeans 豆丝 with garlic 大蒜 and a generous amount of chilli 辣椒, all soaked in a swimming pool of vegetable oil 植物油, yummy.

yeah its everywhere(in sichuan anyway, and local brand). usually in bottles or sealed plastic cups. its called 豆奶dou4nai3 or 豆浆dou4jiang1.you can get it hot or chilled.
alternatively you can get it freshly sqeezed every morning at any 包子店bao1zi dian4， shops that sell vegetable and meat filled dough buns. a popular breakfast for many locals.
