女士nǚshì should be translated into Madam, and 小姐xiǎojiě is Miss. 

I won't use the word 小姐xiǎojiě in China when asking the waiter for help since it means prostitute. But in other Chinese speaking places, it is okay to call waiters by 小姐xiǎojiě or 先生xiānshēng.

别怕黑bie2pa4hei1 is the way to tell other people not to be afraid of darkness, for example, 你别怕黑,有我在。

不怕黑bu2pa4hei1 is the way to express oneself that he/she is not afraid of darkness, for example, 我不怕黑,你抓着我吧。

It is usually used in a negative situation. Note that it is different from the word 坚持.
