The doctor will either ask " Do you have any allergies?" (你有没有任何过敏，Nǐ yǒu méi yǒu rènhé guòmǐn?)。你(you)有没有(represents "have" in the question sentence)任何(any)过敏(allergy)?
Are you allergic to anything? (你有没有对任何东西过敏?Nǐ yǒu méi yǒu duì rènhé dōngxi guòmǐn?) Translation: 你(you)有没有(means " have" in the question sentence)对(to)任何东西(anything)过敏(allergy)。

October 1, 2010 at 3:03 PMIn general ( daily conversation), both of them are right and has the same meaning. Strictly speaking, 你怎么不打电话给我？will give a relatively negative feel. It sounds a little bit of accusation. On the other hand, 你怎么没打电话给我? gives a feel that the person is just simply asking a question without blaming. But it also has something to do with the way / ton of the person who speaks. But if you are not sure the situations/what's happening, I would suggest you to use 你怎么没打电话给我?

Does 充值 only refer to money on a phone card?
Here in Belgium we have a Proton card that we only use for shopping and also can be recharged.
So, we have to specify which card we recharged.
